These recent advances typically provide significant results with minimal invasiveness and minimal downtime. One of the most recent technologies addresses a common patient concern, excessive sweating.
Abnormally excessive sweating under the arms, not necessarily related to exercise or heat, is called primary axillary hyperhidrosis. According to a 2004 study in the Journal of the American Academy of Dermatology, it is estimated that approximately four million people in the United States have primary axillary hyperhidrosis (Strutton, 2004).
Axillary hyperhidrosis can be distressing to the patient and can affect their lifestyle, confidence, and activities. To seek relief from excessive sweating, patients have taken anticholinergic oral medications, multiple injections of botulinum toxin, and even surgical procedures (endoscopic thoracic sympathectomy).
Fortunately, there is a nonsurgical, permanent solution with immediate results for axillary hyperhidrosis, and it is called miraDry. miraDry uses microwave energy to selectively destroy sweat glands (thermolysis). Specifically, miraDry targets both types of sweat glands, eccrine as well as the odor-related apocrine glands. By targeting both types of sweat glands, miraDry improves both odors as well as wetness. In fact, a study revealed an 83% reduction in sweat production at a seven-month follow-up, and a 93% reduction in odor (Lee, 2013). Another study published in April 2012 from the University of British Columbia reported that over 90% of patients experienced a reduction in sweating, and the average sweat reduction in each participant was 82%. The satisfaction rate of study participants was 90% (Hong, 2012). Although the miraDry procedure provides immediate and lasting results, a minority of patients, typically those with significant hyperhidrosis, do undergo the miraDry procedure a second time.
Prior to the treatment, a complimentary consultation is performed to review your concerns, discuss your treatment options, as well as the details of the miraDry procedure to see if it is right for you. Photographs are taken of the treatment areas prior to the procedure.
How do you prepare for the miraDry procedure? You’ll be asked to shave both underarms four to six days before the day of the procedure. Shaving less than four days prior to the procedure will results in hairs so short that it will be more challenging to identify the treatment area. Wash your underarm area with Hibiclens solution while you shower daily for one week prior to the miraDry procedure. Hibiclens cleanser is available without a prescription at your local pharmacy. Also at your local pharmacy, purchase ibuprofen and/or acetaminophen to take later for post-procedure swelling and soreness. Additionally, freeze four small water bottles to use as ice packs after the procedure. Pura Vida Medical Spa will also provide ice packs on the day of your procedure. On the day of your procedure, preferably wear a dark tank top with loose armholes. If you wish, Pura Vida Medical Spa may provide one for you.
Your miraDry treatment day has arrived! The miraDry procedure itself involves three steps, and the total treatment time is 60-90 minutes. The first step involves numbing the treatment area by injecting a lidocaine solution. Next, the treatment area, which is customized to your unique body, is marked using a temporary tattoo. Once you are adequately numb, the miraDry handpiece is then applied to the treatment area. Gentle suction stabilizes the treatment area and ensures an accurate and reliable treatment depth. Thermal/microwave energy is then delivered which selectively and permanently destroys the sweat glands. Simultaneously, the miraDry handpiece provides cooling to the superficial tissues, making the procedure more comfortable as well as protecting these areas superficial to the sweat glands. Most miraDry patients describe the treatment itself as painless, however, an occasional “hotspot” may occasionally be felt.
After the miraDry procedure, you may experience swelling, numbness, bruising, and sensitivity in the underarm area for several days to weeks after the treatment. These anticipated findings self-resolve as time passes. You may also feel lumps in the treatment area. If you feel these lumps, it is recommended that you gently massage these areas multiple times a day to speed up their resolution. It is recommended that you continue the use of Hibiclens cleaning of the treatment area during your daily shower for an additional week.
After your miraDry treatment, you will experience immediate and lasting results, and you will continue to perspire normally elsewhere. As the lidocaine solution that was infused prior to the procedure drains over the next several days, it may be mistaken as sweat until it also resolves. Although most patients are able to return to regular activity (like returning to work) immediately, it is advised to schedule the miraDry procedure with a light schedule in the days following the treatment. Your exercise routine is typically resumed several days after your procedure date.
Now that you’ve undergone the miraDry procedure and are enjoying the results, will your body compensate by sweating more elsewhere? The answer is “no.” Your body has two to four million sweat glands in areas including the palms of the hands, soles of your feet, and the cheeks and forehead of the face(sweathelp.org). Only 2% of your sweat glands are in the miraDry treatment areas. Therefore the other 98% of the sweat glands will function as they did previously. Also, the other tools your body uses for thermoregulation, including vasodilation or constriction, continue unchanged post-procedure.
To date, there have been over 300,000 miraDry procedures performed to date, and miraDry is currently available in over 50 countries. miraDry received its first FDA clearance (and CE-marking in Europe) in 2011 for treating axillary hyperhidrosis (excessive underarm sweating). Later, in 2015, miraDry was FDA-cleared to permanently eliminate underarm hair of all color, as well as odor glands (the apocrine glands). With these additional FDA-clearances, miraDry was recognized to effectively address the trifecta of underarm sweat, odor, and hair.
